Our patient experience award, recognises individuals and teams, either from the Trust or teams that co-deliver care with partner organisations, who have delivered exceptional patient experience. The individual or team will have delivered exceptional patient experience which went above and beyond, and included delivery of high-quality, compassionate care, and enhanced patient and carer experience. They have shown that patient needs, as well as the needs of families, relatives and carers, are fundamental to the way they deliver their care or service, and can evidence that wider teams have been involved when required to improve the delivery of seamless care. They demonstrate the value ’we put our patients first’ - focusing on the patient to provide high-quality care and a service that is tailored to the individual.
